Document Can I provide a mix of NHS and private dental treatment on the same tooth?

Providing both NHS and private treatment options have been discussed, and the treatment isn't available under NHS regulations, performers are able to provide a mix of NHS and private dental treatment.

The patient should be issued with a FP17DC form or equivalent, documenting the NHS and private elements being provided under the planned Course of Treatment.

Orthodontic provision however, must be entirely private or entirely NHS.
