Document What is a D7 / D7A form?


A D7 is the form a performer uses if they'd like to request a second opinion from a Clinical Advisor, either because they're having a clinical disagreement with a patient or they would like help with a treatment plan.


A D7A is the form that a Commissioner (Area Team / Local Health Board) or NHSCB (Commissioning Board) would use to request a patient examination by a Dental Reference Service (DRS) clinician for situations where an independent clinical opinion is sought. This is usually when local resolution of a complaint hasn't been successful or where assessment of the appropriateness and quality of treatment is required.
