Pharmacists can act as a patient’s representative and can sign a prescription form to claim exemption on the patient’s behalf, however to do so they must have seen proof of the patient’s entitlement to exemption from prescription charges. 
 
They should not sign a prescription form to claim exemption on the patient’s behalf if they have not seen proof of the patient’s entitlement to exemption from prescription charges. 
 
A pharmacist cannot sign the form as the patient’s representative while also marking the form as ‘evidence not seen’.
