Document What can I do if an exempt patient has an EPS prescription but there is no token to sign?

A patient’s declaration must be captured on the appropriate form. For ‘electronic’ prescribing the appropriate form is the ‘Token’ – this may be the Prescribing Token (FP10) or the Dispensing Token (FP10DT).
 
Please note that we require tokens to be submitted for all declarations of charge paid or exemption other than those who are classed as age exempt; ie 60 and over or under 16 whose date of birth has been inserted into the prescription message by an appropriate computer system that stores the patient’s NHS care record.
