Document What is the procedure for updating your records when a pharmacy closes?

The Area Team (AT) will need to complete a form on the contractor’s behalf to inform us that a pharmacy has closed.

It is also useful if the contractor can inform us of any new correspondence addresses and if they can leave the bank account active until all payments have been made.
