Document What is the procedure for updating your records to amend a pharmacy’s details?

The Area Team (AT) will need to complete a proforma on the contractor’s behalf to inform us of any changes to the pharmacy.
They can complete this form to notify us of the following changes:
  • Change of owner name e.g to a Ltd company.
  • Change of trading name.

    Change of responsible Area Team.

  • Minor relocation.
  • Change of telephone number.
