Document What is the procedure for updating your records when an appliance contractor changes ownership?

Please note that if a change of ownership occurs where a company has bought a business on a non debts and liabilities basis, we will issue a new contractor code.

If a change of ownership occurs and the contractor completely buys out another company including debts, liabilities and/or access to bank account, we will not issue a new contractor code.

To inform us of  the change of ownership, an authorised signatory at the Region will need to complete a proforma on the appliance contractor’s behalf
to inform us of the change in ownership.

They would return the completed form to nhsbsa.prescriptioninformation@nhs.net
