Document I have recently set up a new service and have not received an FP34PD claim form this month; what should I do?

If your service holds a GMS (PMS or APMS) contract we hold that information on our systems and this will automatically generate an FP34PD form allowing you to claim payment for the ‘dispensing’ of the items that you have personally administered and are covered by the GMS Statement of Financial Entitlements.
 
If you hold one of these contracts but have not received an FP34PD then please contact our helpdesk.
