To allow to process your application, please return the PPC06 letter with a copy of the receipt issued by the pharmacy and a covering letter containing the following information:

  • The pharmacy's name
  • The pharmacy's full address (including postcode)
  • The pharmacy's telephone number
Our address is as follows:

You can also send us this information via email at nhsbsa.ppc1@nhsbsa.nhs.uk.

Alternatively, you can call us from the pharmacy you purchased the PPC at on 0300 330 1341 so that we can confirm the purchase with both parties.
