Document What evidence is required to claim a refund?

The evidence required depends on the type of charge that you're claiming a refund of:
  • NHS prescriptions - Original FP57 receipt (only available at the time of purchase)
     
  • NHS dental treatment - Original NHS receipt(s)
     
  • Sight test - Original receipt
     
  • Optical voucher towards cost of glasses or contact lenses - Original receipt(s) and a copy of your optical prescription
     
  • NHS wigs or fabric supports - Original receipt(s)
     
  • Travel costs - Original receipt(s) (If no receipts were available for your form of travel, please include a covering letter to explain why)
If you're claiming a refund of dental, optical or NHS wigs and fabric supports costs and don't have an original receipt, you'll need to go back and obtain a duplicate.
