Document What is the purpose of form TV18 Annex DC?

The TV18 Annex DC letter is sent out to any NHS Pension Scheme member who has returned a TV18 option form confirming that they wish to transfer their NHS pension benefits to a defined contribution (DC) pension scheme. The letter advises:

The transfer value includes guaranteed minimum pension (GMP) and / or post 5th April 1997 contracted out salary related (COSR) rights (Section 9(2B) rights). As payment is being made on or after 6th April 2012 following the abolition of contracting out on a defined contribution (DC) basis, the member is required to sign the enclosed declaration form TV18 Annex (DC) before payment can be made.

If you have been sent this letter along with the accompanying declaration form consider the information provided carefully. If you wish to proceed with the transfer of your pension benefits sign and date the declaration form then return it to us as soon as possible.
