Document Can an application form be signed on behalf of a member if they’re unable to do this themselves?

In the event of a member being unable to physically sign their own application form, we will still accept a form if the member has made a mark on it in the presence of a doctor, with the doctor confirming that the member is aware of what the mark is for.
If the member is unable to make a mark on the application form, only a person who has an Enduring Power of Attorney in respect of a member is allowed to sign forms on their behalf.
