Document Which application forms should I complete to be considered for retirement on the grounds of ill health?

To be considered eligible to retire on the grounds of ill health, please complete the following forms:
  • :Consideration of entitlement to ill health retirement benefits (AW33E) – if you are an active, contributing member of the NHS Pension Scheme (obtained from your employer) or if you are in NHS employment but you have opted out of the NHS Pension Scheme.
     
  • Consideration of entitlement for early payment of deferred benefits due to ill health (AW240) – if you are not in NHS Employment and a deferred member of the NHS Pension Scheme (obtained from the NHS Pensions website)
If you are not currently an active member of the NHS Pension Scheme, you will only be eligible for payment of Tier 1 pension benefits if your application is accepted.

If your application is accepted, the next step is to complete one of the following forms to apply to take your pension benefits:
  • Retirement benefits claim form (AW8) – if you originally completed an AW33E (obtained from your employer)
     
  • Deferred benefits claim form (AW8P) – if you originally completed an AW240 (obtained from the NHS Pensions website)
If you have been informed that you have a limited life expectancy of one year or less, and would like to request that all of your pension benefits are paid as a one-off lump sum, please also complete an Application to commute ill health retirement benefits (AW341). If you are over your normal pension age, please complete an ‘Application to commute age retirement benefits on grounds of serious ill health’ (AW341a).
