Document I have indefinite leave to remain in the UK, how do I apply for an EHIC?

Applying for the first time:

If you have indefinite leave to remain in the UK and you have not had an EHIC before, you would need to apply for an EHIC via post including evidence. We would accept Home Office documents or a UK Visa stating "indefinite leave to remain”, UK citizenship granted etc.

You can find the application form on the NHS website, you should return it to us at the below address along with your evidence:

Bridge House,
152 Pilgrim Street,
Newcastle Upon Tyne,
NE1 6SN
Renewal:

If you already have an EHIC which has expired (not more than 2 years ago) and you sent us your evidence of indefinite leave to remain in the UK the first time; and you permanently reside in the UK, you can apply by using one of the following methods:

  • Online

  • By telephone on 0300 330 1350 if you are calling from the UK.

  • By telephone on 0044 191 279 0575 if you are calling from outside of the UK.

  • By post. You can find the application form on the NHS website. Once completed, please send your application to:
