Document What is the 'Annual Declaration of Earnings' for Tier 2 ill health?

If you are in receipt of a Tier 2 ill health pension every April you will be asked to complete an ‘Annual Declaration of Earnings’ form. We will send this to you, along with a postage paid envelope.

You will need to complete this form and return it to us even if you have not worked. If you have worked and are unsure what information we need you must send in a copy of your payslips along with the form.
