Document What can I do if I've sent in my FP57(s) but it appears to have got lost in the post?

In order for us to consider a refund you should contact the Good Cause Reviewers (PSQ) explaining that you have not received the FP57(s) back. Your letter should include the date of the FP57 and the amounts paid.
