How you claim a refund of NHS dental charges depends on which country you paid for your treatment in.
 

England

To apply for a refund of NHS dental charges paid in England you need to complete an HC5(D) form which can be ordered / downloaded from our website.

You need to attach the original receipt(s) which need to include the amount paid and the date that the payment was made (we cannot accept treatment plans or credit / debit card statements as proof of payment).

Part 4 of the HC5(D) form tells you where to send it to, depending on which exemption you have.

 

Scotland

To apply for a refund of NHS dental charges paid in Scotland, you need to complete an HC5 form which can be downloaded from our website.

If you're unable to print off the HC5 form, you can either contact the Practitioner Services or collect one from your local GP surgery or community pharmacy.

Wales

To apply for a refund of NHS dental charges paid in Wales, you need to complete an HC5W(D) form. 
You can download this from the NHS Wales website.
 
To obtain a HC5W(D) form you can either order one by calling the following number and select the option for ‘health publications’ or pick one up at your local GP surgery, community pharmacy, NHS dentist, optician or Jobcentre Plus office.

Northern Ireland

To apply for a refund of NHS dental charges paid in Northern Ireland, you need to complete an HC5 form.
 
You can collect the form from your Social Security Office or Job and Benefits Office.
 
You need to attach the original receipt(s) which need to include the amount paid and the date that the payment was made 
(we cannot accept treatment plans or credit / debit card statements as proof of payment).
