Document Can I supply a missing medical condition over the phone?

No, you cannot supply this over the phone.

If no medical condition has been provided we'll send out either a MED03 or MED05 letter:
  • MED03 - this letter is sent to the GP if they've signed the FP92A application form but haven't indicated a medical condition. They need to complete the medical condition section and send it back to us.
  • MED05 - this letter is sent to the applicant if the FP92A application form hasn't been stamped by the GP and no medical condition has been indicated. The patient should ask their GP to complete the medical condition, stamp the relevant section and send it back to us to process.
