You need to complete an HC5(W) form to apply for a refund of NHS wigs and fabric support charges paid. You can download a HC5(W) form from our website. The HC5(W) form tells you where you need to send it to.

To claim a refund you need to send in the original receipt(s) with your HC5(W) form; the receipt needs to include the amount paid and date that the payment was made.

Scotland

If you live in Wales and have paid to receive an NHS wig or fabric support in an English NHS trust area, you may be able to claim a refund via your Local Health Board (LHB).

 

Northern Ireland

You need to complete an HC5 form to apply for a refund of NHS wigs and fabric support charges paid. You can obtain a HC5 form in person from your local Social Security Office or Job and Benefits Office. The HC5 form tells you where you need to send it to.
