Document What documents could I show to my pharmacy to prove I am entitled to free NHS prescriptions?

The following documents could be used as proof of exemption:

 
Reason you are exempt
Evidence
You get, or are included in an award of someone getting, Income Support
Income Support award letter
You get, or are included in an award of someone getting, Pension Credit Guarantee Credit
Pension Credit award letter
You get, or are included in an award of someone getting, Income based Jobseekers Allowance (JSA)
Jobseekers Allowance award letter
You get, or are included in an award of someone getting, Income Related Employment and Support Allowance (ESA)
Employment and Support Allowance award letter
You get, or are included in an award of someone getting, Universal Credit and meet the additional criteria
Universal Credit statement
You are under 16 years old
Birth certificate, passport or medical card
You are 16, 17 or 18 years old and in full time education
Child benefit award letter, or birth certificate / passport / medical card and proof you are a full time student
You are 19 years old and included in an award entitling your parent / guardian
Your parent / guardian’s Tax Credit award notice that includes you as a dependant and shows an income level of £15,276 or less, or;
Your parent / guardian’s qualifying benefit award letter that includes you as a dependant, or;
Your parent / guardian’s Universal Credit statement that includes payment for a dependant and and proof you are a full time studentand proof you are a full time student
You are 60 years old or over
Birth certificate, passport or medical card
You are entitled to, or named on a valid Tax Credit Exemption certificate
Tax Credit Exemption certificate or Tax Credit award notice
You are named on a valid Medical Exemption (MEDEX) certificate
Medical Exemption (MEDEX) certificate
You are named on a valid Maternity Exemption (MATEX) certificate
Maternity Exemption (MATEX) certificate
You are named on a valid HC2 certificate
HC2 certificate
You are named on a valid War Pension Exemption certificate
War Pension Exemption certificate


It is your responsibility to ensure you are exempt if claiming free NHS prescriptions. If your pharmacy is ever dissatisfied with the evidence you show them, they should mark the prescription as ‘Evidence not seen’.
