Document What information is required when completing a BUR101R form?

The BUR101R form is a single page document which requires all of the following information:
  • Students full name and address
  • Students date of birth
  • Students reference number (if known by HEI)
  • Name of HEI
  • Name of course (e.g. Diploma in adult nursing)
  • Name of commissioning Local Education Training Board (LETB)
  • Confirmed return date
  • Cohort joining (In the form MONTH/YEAR)
  • End date of course
  • Details of completing HEI officer and date completed
Due to the Health Education Funding reforms, if a student resumes to a course on or after the 1st August 2017 in a cohort which started on or after the 1st August 2017 any commitment to a bursary package can only be made by Health Education England (HEE). When sending the BUR101R you should also attach a copy of the HEE approval email along with the BUR101R.
