Document recently updated When I downloaded the Community Pharmacy Flu Vaccination Service payment claim form it printed out on two sheets of A4 paper. Can I submit this form?

18/19 Flu Season

No. Pharmacy contractors making a claim for payment must use the form available on our website, which has been formatted to print on one side of A4 paper so that it is accepted by the NHS Prescription Services scanning machines. Claim forms downloaded from other systems which are not formatted in this way will not be accepted.

19/20 Flu Season


Paper claim forms cannot be accepted for this flu season and contractors must submit their flu claims using the Manage Your Submission (MYS) portal.
